William was enrolled in program in October of 2024. His weight was 218.5 pounds. His current weigh is 142.4 pounds. The patient has lost a total of 76 pounds in the last 10 months. Patient also reported that his HA1C was at 10 and it is now 5.2.

Ronald had BG of over 200 regularly (up to 250- 260) and is now seeing BG of 130- 140 since following the diabetes plate model method, with no low BG and only one instance of high BG within 1-2 months since we last spoke. He used to have 250- 260 blood glucose daily. He has taken the diabetes plate model seriously and attributes his better glucose numbers to this.

Cathy is a patient from Dr. A. Since she started the program in May, she has lost around 6-8 lbs AND her recent blood work from the end of August shows that she was able to lower her total cholesterol from 263 to 211, her triglycerides from 282 to 189, her LDL from 165 to 129, and her liver enzymes came back to normal range. She still has some work to do but according to Cathy, Dr. A was very pleased with her results and with the work of the program. She is still hoping to lose more weight and to lower those numbers even more. This fall has been a bit rough for her with work demands, but she has blood work again in December. Fingers crossed for even lower lab values!! Yay to Cathy for her hard work!

Christopher with Dr. S had unstable blood sugar in general- experiencing some lows, and highs. Our last conversation, we discussed the 15/15 rule, carb counting (how to), timing of meals, and sources of carbohydrates to prevent and treat low BG with specific grams of carbs being recommended. He has followed these recs and since this visit he’s had zero incidents of low blood sugar. He also received education on how to address higher blood sugar levels- he was experiencing high BG of 200- 250’s, since the conversation on how to lower blood sugar occurred, he has not had these numbers and the highest (per one occasion) was 180- 181. He is having far more normalized blood sugar levels now!

Erwin struggled with rainout (waking with water on his face from CPAP mask) and mask seal issues causing leakage. A humidity change request was sent, and the patient was also recommended to use facial moisturizer and facial hair conditioner to improve the seal. The moisture acts similar to glue. With these adjustments, leakage is within normal ranges, and the patient reports sleeping very well.

Patient Teresha has struggled with her weight; she is morbidly obese and suffers from uncontrolled diabetes and hypertension. As soon as we signed on with RPM Logix, I knew Teresha would benefit from the dietitian/CDE consult, and she was very happy to sign on to this. Her dietitian has done a wonderful job of working with Teresha and communicating with me regarding needs for medication changes, and when there have been issues that the provider needed to be involved with. Due to this excellent nutritional care her dietitian has provided, Teresha has lost 47 lbs over a few months, is much more motivated for proper food choices, and to exercise. For the first time, her BP and glucose are in control, and she feels so much better. I have really praised Teresa for her hard work today, but also want to praise her dietitian for her dedication and for making such an improvement in this patient’s life.

The patient has been enrolled in the Sleep Buddy Program since December 2024 following a diagnosis of severe obstructive sleep apnea. At the time of diagnosis, her sleep study demonstrated an AHI of 46.4. She was prescribed CPAP therapy and has maintained compliant with treatment. In addition, she was prescribed GLP-1 medication to support weight loss as a means of improving sleep apnea. To date, the patient has achieved a weight loss of 40lbs and maintains an average AHI between 0.1 and 0.5!

I have a very motivational story to share of one of my patients Jacinta whom I have had the privilege to get to know over the past 6 months. Jacinta has done an amazing job with her weight loss journey and now has officially reached her goal. She is 194 lbs down from 221 lbs 8 months ago. A total of 53 lbs total since November 2021. Her BMI in January was 37.92 and now it is 28.6, reversing her “obesity” classification. Jacinta followed a strict regimen of eating mostly vegetables, intermittent fasting, and physical activity. She is proud to have gone down 6 dress sizes and now needs to have her entire wardrobe adjusted. She remains motivated to maintain her weight loss and promises to stick with her regimen. We wish Jacinta all the success with her maintenance phase as she moves forward!

John started CPAP therapy on 12-03-24. Usage/compliance was 43%, lower than insurance coverage guidelines and considered non-compliant. He is not feeling well rested, has daily fatigue, daytime sleepiness, naps daily, and has brain fog. Patient is now compliant at 86%, AHI within normal limits, and leakage within normal ranges. Patient is now feeling more well rested, fatigue has greatly improved, no daytime sleepiness, and no brain fog. Patient is stable!!
